Care Management Program
Help Unlimited's Care Management Program is designed to complement our caregiver services. It offers oversight and coordination of medical, social, legal, and emotional needs when family members are not able to provide it.

Help Unlimited Care Management can:

• Assess the level of daily functioning, physical and emotional health, financial means, safety factors, etc.
• Develop and implement a personalized Plan of Care, coordinating all care professionals involved.
• Identify the need for and procure adaptive equipment.
• Keep family members informed on a regular basis.
• Provide long term solutions for care, keeping in mind the physical, psychological, social and financial needs of the client.
• Coordinate financial needs (iei.e.: petty cash, banking, bill paying, etc.)
• Provide advocacy for doctor, dental and other appointments and interface with other professional care providers.
• Provide referrals to attorneys, physicians, and other professionals for legal, fiduciary and medical needs.
• Coordinate household services, such as plumbers, electricians, gardeners, etc.
• Manage medications and facilitate compliance.
• Assist with health care advance directives.
• Provide placement services in the most appropriate facility.
• Monitor care and provide patient advocacy in facilities.
• Insure the highest possible quality of life for your loved one.
• Facilitating home telehealth.
• Provide other services as needs occur.
